LOYOLA General Study Abroad application Guidelines
(for IES, USAC, SIT, Affiliate, Exchange, & Petition Programs)
ALL materials on the Application Check List below must be received before Loyola will approve you to study abroad. Loyola's approval is required in order for your study abroad program to accept you.

Fill out and submit to the Office for International Programs (OIP). |
| Program-Specific Application |
IES - Apply online. Request through LOCUS that your official Loyola transcript be sent directly to IES. IES may also require that additional application materials be sent directly to them (e.g. IES Foreign Language Assessment form). All IES applicants must fill out the top of an IES Authorization & Evaluation form and submit it to OIP. USAC - Apply online. Request through LOCUS that your official Loyola transcript be sent directly to USAC. Submit the $200 deposit to OIP -- DO NOT submit an additional $200 deposit directly to USAC. SIT - Apply online. Request through LOCUS that your official Loyola transcript be sent directly to SIT. Additional application materials must also be submitted directly to SIT. Note: SIT charges a $50 non-refundable application fee. Affiliate (Chile) - Fill out a hard copy of the Chile application and submit to OIP. Request through LOCUS that your official Loyola transcript be sent to OIP. Affiliate (El Salvador) - Follow the Casa application instructions listed on Santa Clara University's website. Exchange - Make an appointment with Kelly Heath in OIP to receive instructions on your exchange program-specific application. Request through LOCUS that your official Loyola transcript be sent to OIP. Petition - Contact your program provider for their program-specific application instructions |

Write a one-page essay discussing why you would like to participate in this program in terms of your background, interests and goals. Submit to OIP. |
| $200 Deposit |
Payable by check, made out to "Loyola University Chicago." Your deposit goes towards the cost of the program. Submit to OIP. |

Part I of this form must be completed by your OIP advisor during an appointment or walk-in hours. Part II of this form is to be completed by the student. Part III of this form is to be completed by your academic advisor/dean. Follow instructions listed on the form regarding how to have Part III completed (instructions vary by school and class year). |
LOYOLA Summer Programs application Guidelines
(for Faculty-Led Programs: France, Spain, Tunisia; Service Learning Program: Peru; and The John Felice Rome Center)

| Passport |
A photocopy of a vaild passport or a passport application receipt must be included along with the application for the Faculty-Led Programs and the Service Learning Program. For Rome, only a copy of a valid passport will be accepted. |
| Official Transcript |
(For visiting students ONLY) An official transcript must be included. |
| Deposit |
A $100 deposit is required for those traveling to France, Spain, Tunisia, or Peru. This deposit covers the non-refundable OIP fee. A check or money order should be made payable to Loyola University Chicago. For those traveling to Rome, a $500 deposit should be made payable to Loyola University Chicago which includes the $100 non-refundable OIP fee. |
