Rates and Terms of Coverage

STUDENT HEALTH INSURANCE, 2011-2012 SCHOOL YEAR

Loyola's student health insurance provider for the 2011-2012 school year is United HealthCare Student Resource.  The provided rates apply to Loyola students, their spouses or domestic partner, and children. The student's annual or spring charges will be billed to his/her Loyola student account if he/she meets the eligibility requirements.  All other charges should be paid directly to United HealthCare.  To view the 2011-2012 insurance plan brochure, visit www.UHCSR.com/luc

STUDENT HEALTH INSURANCE, 2012-2013 School Year

Loyola's student health insurance provider for the 2012-2013 school year will be administered by Academic HealthPlans and is underwritten by Blue Cross and Blue Shield of Illinois.  Students may visit the plan website at http://www.AHPCARE.com/luc.  Information such as the plan benefits brochure will be posted there as it becomes available.  If you do not meet the automatic enrollment eligibility below, but would still like to enroll in Loyola's Student Health Insurance, you may do so by directly enrolling with Academic HealthPlans at http://www.AHPCARE.com/luc.  

Fall Insurance Waiver Deadline is October 1, 2012
Spring Insurance Waiver Deadline is February 28, 2013

Eligibility Requirements

Loyola University Chicago's Board of Trustees requires that all undergraduate students enrolled for 12 or more credit hours and all graduate and professional students enrolled for 8 or more credit hours (or registered for thesis or dissertation supervision) at the start of the fall or spring semester have individual health care insurance.

All students meeting the eligibility requirements will be charged for Student Health Insurance by the University on their student account and enrolled in the plan, if a waiver is not submitted before the deadline date. If you already have your own insurance or are covered under a parent's plan, you may waive out of Loyola's Student Health Insurance by providing proof of other coverage. If your account has been billed for the insurance, a credit for the insurance will post to your student account within 48 hours.

In the event that a student does not meet the eligibility requirements to be automatically billed by the University for Loyola's Student Health Insurance for the 2012-2013 school year, but would still like to purchase coverage, he or she can do so by visiting Academic HealthPlan's website at www.AHPCARE.com/luc or by calling 1-855-AHP-CARE once plans become available for purchase. 

    

Coverage Options

Parents/Guests, who have been granted access rights, are now able to waive out of or fast track into Loyola's insurance plan on behalf of the student.

How To Waive Out of Loyola's Student Health Insurance:

If you have your own insurance already or are covered under your parent's plan, you may waive out of Loyola's Student Health Insurance by providing proof of the other coverage.  

  1. Log on to LOCUS and click on "Student Health Insurance" in the "Campus Finances" section. 
  2. Submit the electronic waiver by October 1st for the Fall 2012 term (February 28th for the Spring 2013 term).
  3. The insurance charge will be waived from your account.

How to Fast-Track Into Loyola's Student Health Insurance:

If you want to enroll in Loyola's Health Insurance and already have the charge on your student account, you may accept or Fast-Track into the plan by logging into LOCUS and clicking on the "Student Health Insurance" link in the "Campus Finances" section. Fast-Tracking confirms that you have accepted coverage through Academic HealthPlans, activated your enrollment into the plan, and requested a HealthCare ID card to be generated prior to the October 1, 2012 Fall waiver deadline (February 28, 2013 Spring).

How to Purchase Supplemental Insurance Coverage:

Insurance coverage for yourself, a spouse or domestic partner, or a child can be purchased directly from United Healthcare Student Resources for the remainder of the 2011-2012 school year. 

In the event that you did not meet the eligibility requirements to have been automatically billed for Loyola's Student Health Insurance by the University, but you would like to purchase summer, monthly, or continuation coverage, you can do so by filling out the enrollment form on United HealthCare's Website at www.uhcsr.com/luc or by calling them at 1.800.964.4698.

To add a spouse, domestic partner or child onto your insurance plan, you can purchase supplemental coverage by filling out the enrollment form on United HealthCare's Website at www.uhcsr.com/luc or by calling them at 1.800.964.4698.
