Completed Printing Request Forms will be routed via the internet to Anastasia Lowery, PBMS Customer Operations Manager or Ruth Quint, Procurement Specialist.

Forms Management
The Printing Services Department produces and distributes forms that are not online. For those forms that are online, the department also determines compliance of forms to the University's identity program. The program is responsible for consolidating and reducing the number of forms to maintain efficiencies and generate cost savings for the University.

The owner of copyrighted work has the exclusive right to duplicate and distribute copyrighted materials. In order to assist instructors in obtaining this permission, Printing Services has implemented a procedure by which permission may be sought and course packs prepared. All faculty and staff must follow these procedures in order to provide protection to the University from civil and criminal penalties. You can begin this process by completing the Copyright Clearance form online.
