The following forms are provided as a courtesy to members of Halas Sports Center. You will need to have an Adobe Reader in order to view and print these forms. Fortunately, Adobe Reader is available as a free download from Adobe. Click HERE to download the latest version of Adobe Reader.

This form MUST be filled out when signing up for an Intramural Event. Print out the form, fill it out completely, and then drop off the form at the front desk of the Halas Sports Center. |

Use this form if you are an individual looking for a team or if you are a team looking for additional individual players. After filling out the form completely, bring it to the front desk of the Halas Sports Center. |
