This page MUST be filled out when signing up for an Intramural event. To sign up for Individual Events, fill out this page only. To sign up for Team Events, you must fill out Page One AND Page Two.
Intramural Entry Form - Page Two
This page MUST be filled out when signing up for all Team Events for an Intramural event. After filling out the forms completely, bring them to the front desk of the Halas Sports Center.
Use this form if you are an individual looking for a team or if you are a team looking for additional individual players. After filling out the form completely, bring it to the front desk of the Halas Sports Center.
