IMPORTANT - PLEASE READ 

EARLY STOP FEE: if the payroll deduction option is stopped before the ONE YEAR ANNIVERSARY of signing up, a $50 Early Stop Fee will be deducted, along with your final deduction.

I hereby authorize Loyola University Chicago to discontinue my payroll deductions for my Halas Sports Center Membership fees.  I understand that a $50 Early Stop Fee will be added to my final deduction if the one year anniversary of my signing up has not been reached.

I understand that my Halas Sports Center privileges and the membership fee deduction will be discontinued until I resubmit the start payroll deduction form.
