If you have not ever filled out a Lake Shore Campus parking application, please click here and apply first. Then come back and fill out this payroll deduction form. I hereby authorize Loyola University Chicago to deduct my annual Lot Upkeep Fee in proportionate amounts of money from my paycheck each pay period.

I understand the Lot Upkeep Fee will continue to be deducted until the stop deduction form is completed and the university parking permit is returned to the parking office.

If this is a renewal of a permit and you wish to register a vehicle not currently permitted through the Parking Office, please stop by the Parking Office to complete the renewal process as well as register your new vehicle and pick up your permit.
