Ideally, all responses should be accurate, based on data gathered from documents and/or consultations with others at your organization. It is estimated that completing the survey will take approximately 20-40 minutes. However, if you have limited time to respond, and do not have all relevant documentation, we ask that you still complete the survey as accurately as possible, but note whenever you respond using estimates. This information will still be valuable to us.
If you have any questions regarding the survey, please contact Samantha Thoma at samthoma24@gmail.com or Linda Rio Reichmann at lindarr@comcast.net. If you have any technical difficulties with the survey program, please contact Catherine Lee at clee5@luc.edu.
