2008-2009 OSFA Forms

In order to make the verification and appeal process simpler, OSFA has enhanced our forms to allow you to enter the information online. However, forms still require a signature in order to be processed. Use the highlight fields button in the upper right hand corner of the page to help complete the form online. Once completed, print the form using the Print Form button at the top right of the form. You may then fax, mail, or scan and email the signed form. The fax number, mailing address and email address appear a the bottom of each form.
