Chapter 35: Survivors' and Dependents' Educational Assistance
Chapter 35 Survivors' and Dependents' Educational Assistance (DEA) provides educational benefits for eligible spouses and dependents of certain disabled veterans or those who have died due to active military service for up to 45 months. New Loyola Student (never used Survivors' and Dependents' Educational Assistance benefits before):
Applying
- Complete application and send to the VA. Complete VA Form 22-5490, Application for Survivors' and Dependents' Educational Assistance. Include all information concerning the veteran, such as Member-4 VA Form DD-214 (Separation and Discharge Forms).
- Receive Certificate of Eligibility. Once the VA has processed and approved your application, you will receive a Certificate of Eligibility (VA Form 22-0557) from their office in response to your request. This process may take 45 days or more. Submit a copy of this form to OSFA.
Certification of Enrollment
OSFA will electronically send in your Certification of Enrollment (VA Form 22-1999b) to the VA no sooner than 10 days before the start of the term. The VA will then process the enrollment certification and release payment to you. You will also receive an e-mail from OSFA to notify you that your enrollment information has been sent to the VA.
Enrollment Changes
VA regulations require that you immediately notify OSFA if you change your address or withdraw from any class. Discussing enrollment changes with OSFA before you withdraw from the course may help avoid a potential loss of benefits and/or repayment of benefits to the VA. E-mail vetfinaid@luc.edu for advisement before you withdraw.
Other Financial Assistance at Loyola
Students are strongly encouraged to apply for financial assistance. We recommend that all students file a FAFSA for each school year. You can file this form online or with a paper form. Be sure to list the Loyola Title IV School Code (001710) as a school choice on the FAFSA. In order to be considered for most types of financial assistance, you must be enrolled at least half-time in a degree-seeking program.
New Loyola Student (have used Survivors’ and Dependents' Educational Assistance benefits at another institution):
- Submit Change of Place of Training Form to OSFA. Complete and submit VA Form 22-5495 Request for Change of Program or Place of Training for Survivors' and Dependents' Educational Assistance (available online) to OSFA. The Change of Program Form does not require accompanying documents and does not need to be submitted to the VA. Please also submit a copy of your Certificate of Eligibility (VA Form 22-0557) to OSFA.

Continuing Loyola Student
OSFA will certify your continued enrollment electronically to the VA, beginning 10 days before the term begins. Enrolled students receive an e-mail at their Loyola e-mail address to confirm the certification of enrollment (VA Form 22-1999b) has been transmitted electronically from Loyola to the VA. E-mail our office at vetfinaid@luc.edu if you change enrollment, change programs, withdraw, or if your benefits are going to expire.
