Forms should be completed, signed, and mailed to the Financial Aid Office at 1032 W. Sheridan Rd., Chicago, IL 60660. Forms may also be faxed to 773.508.3397 or scanned and emailed to finaidforms@luc.edu (please note that this is not a secure e-mail address).
