Forms should be completed, signed, and mailed to OSFA at 1032 W. Sheridan Rd., Chicago, IL 60660. Forms may also be faxed to 773.508.3397 or scanned and emailed to finaidforms@luc.edu (please note that this is not a secure e-mail address).

Special Circumstance Appeal (1U) Loyola Guarantee Application If you or your family have experienced a change in finances not demonstrated on the FAFSA, submit a Special Circumstance Appeal and documentation of your situation so that we may re-evaluate your financial aid eligibility. |

Students wishing to file a 2009 - 2010 Dependency Appeal may file a FAFSA online or print a paper FAFSA. Complete the student sections of the FAFSA, then submit the FAFSA, your completed Dependency Appeal and all documentation to Loyola's Office of Student Financial Assistance. |

Lender ChangeTo change your Federal Stafford Loan lender, simply sign a new Master Promissory Note (MPN) with the lender. When your lender notifies us (electronically) that you signed the MPN, we will send you an email asking if you wish to use the new lender. Reply to the email that you do want the new lender and we will certify all future loans to your new lender. |
