This form will generate a change of address to the Loyola Payroll System ONLY. Your health and dental carriers, if any, will also be notified of your address change by the HR-Benefits office.

*If you are a student at the University, you must change your student records address through LOCUS. If you have had a NAME CHANGE please contact Human Resources at 312-915-6175.
