2014 Wellness Initiatives


There will be a brand new Wellness Incentive program for the 2015 year. Please check back in 2015 for more information.




All Loyola University Chicago employees who participate in the Loyola Advantage Blue Cross Blue Shield (BCBS) PPO Health Plan are eligible for a $100 cash wellness reward. In order to be eligible for the cash reward, you have to complete the TWO wellness initiatives listed below.

  1. Biometric Screening
  2. Health Risk Assessment

No form of proof will be needed for the Biometric Screenings or Health Risk Assessments as they will be provided by BCBS. Biometric Screenings received in November 2013 and February 2014 will count towards the 2014 Wellness Incentive Program, no exceptions. Health Risk Assessments can be accessed via BCBSIL Well on Target Wellness Website.

To collect your cash reward, you should complete the form and send to Human Resources Water Tower Campus via email hr-wtc@luc.edu or fax 312-915-7612. Employees have until Friday December 26th, 2014 to submit their 2014 Wellness Incentive Reimbursement form. Theses incentives need to take place from January 2014 to October 2014. Biometric Screenings held in the Fall of 2013 and Spring 2014 WILL NOT count towards the incentive program for  the 2015 year.

2014 Wellness rewards will be paid out on a quarterly basis starting April, 2014 and continuing July 2014, October 2014 and January 2015. Wellness Reward forms that are not submitted before the quarter end will be paid in the following quarter. Payment of Wellness rewards are based on BCBS reporting therefore the quarter that the Wellness Rewards are paid is subject to change without notice.

Rewards will be processed and paid through the payroll system and will appear as part of your paycheck (Note: cash rewards are taxable).

How To Access Well onTargetSM Online Tools

There are two ways to get to Well onTarget and begin using the personalized wellness resources.

1.  Log in to your Blue Access for MembersSM account at www.bcbsil.com and click Well onTarget in Quick Links.


2. Go to http://www.wellontarget.com and use your Blue Access for Members username and password to log in.

First time users of Well onTarget and Blue Access for Members will need to create a username and password to access the Liveon member wellness portal. Select Register Now on the log in page and follow the instructions. Make sure you have your BCBSIL member ID card available as you’ll need to enter some numbers from it.
