OIP will enroll you in the CISI health coverage. Please allow three business days for OIP to enroll you in the coverage. If your trip is an emergency and you need to be enrolled immediately, call Marian Carlson at 773 508 3996.
To enroll click on link below, complete form, click submit. OIP will receive an email with your enrollment information.
Please note: Insurance costs will be paid for up front and then charged back to departments on a quarterly basis.
After enrolling you will receive an email directly from CISI with the following:
- Your insurance card
- The CISI brochure
- A letter for the consulate explaining your health care coverage
- Access to the CISI web portal.
