If you are accompanying a spouse or parent on an international trip for Loyola University, please download this form, and send it directly to CISI. Please make sure that the LUC faculty or staff member has registered for insurance first. Your coverage dates should not exceed the LUC employee's coverage dates.

Dependent Insurance Enrollment Form 

After enrolling you will receive an email directly from CISI with the following:

• Your insurance card
• The CISI brochure
• A letter for the consulate explaining your health care coverage
• Access to the CISI web portal.
