To obtain permission to register for PSYC 300. Practicum in Psychology, print and complete this form. Send the completed form to the Psychology Department in Damen Hall, Room 624.

Notes:
1. All placements must be approved by the faculty member in charge of field placements.
2. It is not necessary for you to find an agency entirely on your own; however, students are responsible for applying to agencies, arranging interviews, and learning what volunteer services are needed by those agencies.
Completing and bringing this form to the department will permit the department to unblock you, but you must register as usual.
Also, final enrollment is contingent on having a letter of recommendation in the Psychology Field Placement files from a Psychology Department full-time faculty member attesting to your promise as an effective volunteer in a field setting representing the department and the university.
The letter must be on file when classes start and is to be addressed to the Undergraduate Program Director, Dr. Anne K. Sutter, Department of Psychology.
