Transcripts
Current students and individuals with LOCUS IDs should request official transcripts through LOCUS. Use the request form only if LOCUS is unavailable. Do not use the request form if you have made your request through LOCUS.
Alumni and former students without LOCUS access may mail or fax their Transcript Request Form to:

Transcript Request Form:No fee is charged for transcripts sent via regular mail. Charges will be assessed for transcripts sent via express delivery. Enclose a check or money order in the amount of $12 per address, made payable to Loyola University Chicago.
