Listed below are frequently requested forms in the Department of Residence Life. Please consult the documentation accompanying each form for a complete list of requirements, eligibility information, and other guidelines or instructions.

Desk Receptionist Application

This application is for students who are eligible for Federal Work Study, who have lived in a Loyola University Chicago residence hall for at least one semester, and who are interested in an on-campus job.

Housing Application and Contract

Any student who is required to or who wishes to live on campus must complete a Housing Application and Contract. This form asks for basic applicant information (name, permanent address, gender, academic status, etc.), name of preferred roommate (if any), building preference, lifestyle preference (e.g., do you smoke?, do you go to bed early?), and meal plan preference.

The Housing Application and Contract is not available online. New students (incoming first-year students and transfer students) will receive their contract from Undergraduate Admission. If you are a new student and have not received your contract or you need another, please contact Undergraduate Admission at: 312.915.6500. Please note that Residence Life cannot send new students a housing contract.

Returning students will receive their Housing Application and Contract during the room reapplication process that occurs around March. If you are a returning student and have not received your contract or if you need another, please contact us. Include your name and mailing address in your message.

Before you submit your housing contract, please note the following:

  • The contract is a binding legal document.
  • All unmarried, full-time undergraduate students with fewer than four completed semesters of study are required to live in Loyola's residence halls, unless they are living at home with a parent or legal guardian.
  • The $500.00 non-refundable pre-payment for returning students must be paid to the university to validate the contract. No assignment will be made prior to payment.
  • All students living on campus are required to have a meal plan.
  • The preferences that you indicate on the Housing Application and Contract will be taken into consideration; however, the Department of Residence Life does not guarantee that your housing assignment will reflect all of your preferences.
   

Information Posting Request Form

Registered student organizations, academic departments and members of the administration may request that flyers or posters be displayed in the residence halls. These flyers or posters must be approved by the Department of Residence Life and may only be posted by Residence Life staff members. The form is also available in the Residence Life Central Office in Simpson Living Learning Center.

Important: This form only applies to the residence halls. Centennial Forum Student Union, academic buildings and other areas of campus have different requirements. Minor Visitor Request Form

The "Request Form for Minor Visitation in the Residence Halls" is used for on-campus residents requesting to have a minor visit them in the residence halls.  The form should be filled out by a guest's parent or legal guardian prior to that minor's visit to the residence halls, and the completed form should be submitted (via fax, mail, in person, or scanned with signature attached to an e-mail) no later than 12:00 pm on the business day (Monday - Friday) prior to the minor's visit.  Please note that the requests are reviewed on a case by case basis.  A Residence Life staff member will contact the student host and/or guest directly with a decision regarding the request.

Request for Exemption from Housing Contract

All unmarried, full-time undergraduate freshmen and sophomores under the age of 21, who will not be living at home with a parent or legal guardian, are required to live in Loyola’s residence halls and apartments.  Driving this requirement is our conviction that a full and rich undergraduate experience is best achieved through involvement in the University community.  Students living on campus report a smoother adjustment to the college experience, become involved in organizations, participate in sponsored events, and tend to have a slightly higher grade point average than non-resident students. 

Students wishing to submit a request for exemption must complete the form and submit it to the Department of Residence Life.  A Review Committee will review the request for exemption and will decide whether or not to approve the exemption. 

Please note your exemption is not approved until you receive official written notification from the Department of Residence Life.  You should consider all requests denied until you are notified your request has been approved. 

Request for Release from Housing Contract

The housing application and contract is a legally binding agreement for the entire academic (fall and spring semesters).  Within 14 days of notification of your housing assignment, you must notify the Department of Residence Life in writing if you wish to cancel the agreement.  After the 14-day period, the agreement becomes legally binding, and the student is responsible for all room and board charges for the entire term of the contract (fall and spring semesters). 

Students wishing to cancel the housing application and contract must complete a request for release form to the Department of Residence Life.  A Release Review Committee will review the request to cancel the agreement and will decide whether or not to approve the request.  A student requesting release should not make any other living arrangements until officially notified that the release request has been approved.   

Students should supply all necessary documentation when submitting a request for release.  Documentation may include, but not limited to, an acceptance letter to student's transferring school,
