Admission Forms for Current Students:

Current Student Applications 

Summer School Petition:

Students should meet with their academic advisor to discuss requests for permission to take courses at another institution.  

Applying for Graduation:

Students who plan to complete their degree requirements in May (Spring) or August (Summer) should complete a School of Communication application by October 1 prior to the Spring/Summer terms in which they plan to complete all degree requirements. 

Students who plan to complete their degree requirements in December (Fall) should complete a SOC Graduation Application by March 1 prior to the Fall term in which they plan to complete all degree requirements.

 The Graduation Application is available online through LOCUS (http://www.luc.edu/regrec/graduation.shtml).

Upon completion of the graduation application, students should schedule a graduation degree audit appointment to review remaining requirements.

Pass/ No-Pass:

E-mail this Pass No/Pass Form to your Advisor. Your Advisor will submit it to the Office of Registration & Records. (http://www.luc.edu/regrec/pdfs/UGRD_PassNoPass_Request.pdf )
