Reactivation: Application & Checklist

Use this application if you applied to Loyola before, or if you were admitted to Loyola University Chicago but did not attend.

  1. Open and print out the application (PDF)

  2. Fill it in (please print neatly or type)

  3. Forward this application to the Undergraduate Admission Office at Loyola University Chicago.

  4. Application deadline: July 1 for the Fall Semester and November 1 for the Spring Semester. Classes begin in January for the Spring Semester. If you have taken courses elsewhere, you are required to submit transcripts.

  5. Please allow four to six weeks for us to process your application and inform you of your admission status.

  6. If you would like to meet with an admission counselor to discuss course selection, please contact the Undergraduate Admission Office at: 312.915.6500 (Water Tower Campus) or 773.508.3075 (Lake Shore Campus). If you have college transcripts, you may wish to bring a copy to aid in advising.

If it has been more than a year since you applied to Loyola and you are transferring fewer than 20 transferrable credit hours, you may need to re-send high school transcripts and test scores.
