Spring 2021 COVID-19 Testing Consent Forms

Dear Students, Faculty, and Staff,
Critical to Loyola University Chicago’s plans to reopen our campuses at reduced capacity in the spring semester is our ability to perform comprehensive surveillance testing of our entire community. To accomplish this, we’re pleased to announce that Loyola is partnering with SHIELD Illinois to administer the saliva-based test developed by the University of Illinois.
In the spring semester, Loyola will operate conveniently located collection sites on all three campuses for our students and employees to participate in this mandatory testing program. Please visit our COVID-19 Testing Program website for detailed information about the SHIELD Illinois saliva-based test, testing frequency, compliance, and most importantly, how you’ll need to prepare to take your first test.
Our mandatory testing program will require that everyone sign consent forms. Instructions for how to access and sign the form will follow in a separate message from COVIDfirstname.lastname@example.org.
Your signed consents are due no later than Friday, December 18; this signature is required in order to create a patient account, schedule your visit, and present yourself for testing. Most Loyolans will be required to receive two consecutive negative test results in order for your Loyola ID to be active to access campus buildings.
