Reminder: Surveillance Testing Consent Forms

Dear Students, Faculty, and Staff,
Earlier this week, we shared updates about Loyola University Chicago's surveillance testing plans for the spring 2021 semester. As noted, all Loyola students, faculty, and staff who plan to spend any time on Loyola property during the spring 2021 semester will need to participate in our testing program. Surveillance testing will be completely free and easy to access, with testing sites located on all three Chicagoland campuses. You can find detailed information about the program here.
Before you enroll, you'll need to sign a testing consent form. Instructions for how to access a unique URL and sign the form should have arrived in a separate message from COVIDfirstname.lastname@example.org. That signed consent is due by 5 p.m. Central time on Friday, December 18. It should only take a few minutes to complete, and doing so now will save you considerable time during your first visit.
In the coming weeks, we will send you more information about activating your MySHIELD account. MySHIELD is the patient portal through which you’ll schedule your visits and receive your test results.
