Loyola University Chicago COVID-19 Compliance Policy (Testing, Vaccination, other)

Testing Time Window: Sunday-Saturday
Testing Requirements
Unvaccinated Individuals
- Must test once per week to meet compliance, Sunday through Saturday. If you are on campus twice or more per week, you no longer have to test twice.
- An uploaded test, approved by the Wellness Center, will count towards compliance.
- A weekly test from an outside source must be updated no later than Sunday evening.
Vaccinated Individuals
- Exempt from testing, unless university guidance changes
- Vaccinated individuals are welcome to test at anytime
Unvaccinated Individuals are students, faculty and staff who:
A) have an approved medical or religious exemption from the mandatory vaccination policy, or
B) have not yet uploaded their vaccine record by the mandatory deadline (August 6 for students; September 21 for faculty and staff).
Vaccinated Individuals are those who are two weeks past their full vaccine series and who have uploaded their vaccine record to Loyola’s Health App.
Quarantine Populations (no access to campus)
COVID-19 Positive = 10 days
Close contacts (unvaccinated individuals) = 7 days, with a negative test
Positives & Close Contacts
Each day, the file of positive test results is received by 11 a.m. This report is shared with the contact tracing team. Campus access is removed for 10 days after a COVID Care Coordinator has confirmed that the patient was informed of their instructions to isolate.
Similarly, each day, a report is received from contact tracing indicating those individuals who were potentially exposed to a positive case, are unvaccinated, and must quarantine. These individuals are notified by the contact tracer and instructed to quarantine. Campus access for these individuals is removed for up to 7 days, with a negative test.
Compliance Analysis
On Tuesday of each week, an analysis is conducted to identify all people who were on campus over the previous Testing Time Window, as recognized by a card swipe, desktop computer login, or Wi-Fi proximity. The number of events over the course of a day result in an indicator of campus presence for that day. Multiple events in one day are viewed as one visit.
The testing results from the previous week are aggregated and compared against the visits list. Any individual whose visits and testing schedule do not match the Testing Requirements falls out of compliance. This leads to the Compliance Protocol.
Compliance Protocol
Non-compliance results in the following:
Students
First instance of non-compliance = Warning
Second instance of non-compliance = Removal of campus access privileges and referral to Office of Student Conduct & Conflict Resolution (OSCCR)
- Note: residential students will always maintain access to their residence hall, a dining hall, and laundry facilities
- Campus access is restored at least 48 hours after one negative test
Third instance of non-compliance = Removal of campus access privileges and referral to Office of Student Conduct & Conflict Resolution (OSCCR)
- Note: residential students will always maintain access to their residence hall, a dining hall, and laundry facilities
- Campus access is restored at least 48 hours after two negative tests
Fourth instance of non-compliance = Removal of campus access privileges and referral to Office of Student Conduct & Conflict Resolution (OSCCR)
- Campus access privileges will remain revoked for the remainder of the semester

Department Supervisor/Dean Responsibilities
Supervisors (staff) or Deans (faculty) will be expected to act on the referral from the Compliance Committee and take the appropriate action according to HR policies on progressive discipline for staff and faculty. It is the responsibility of managers to enforce the University’s COVID Compliance Policy equitably and consistently. HR Managers and Faculty Administration will work with department managers to ensure consistent application of the progressive discipline process for issues of testing non-compliance. Staff employees who are unable to work as a result of their UVID being suspended may elect to use their PTO for pay continuity or to take the time off unpaid.
Progressive discipline for staff should coincide with the UVID deactivation as referenced above:
- First instance of non-compliance: N/A - encourage employee to comply
- Second instance of non-compliance (start of progressive discipline): Oral Warning
- Third instance of non-compliance: Written Warning
- Fourth Instance of non-compliance: Unpaid Suspension (one day)
- Fifth instance of non-compliance: Unpaid Suspension (three days)
- Sixth instance of non-compliance (end of progressive discipline): Termination
*Staff who are already in the disciplinary process for other reasons may elevate to a higher level of progressive discipline than identified for non-compliance.
