  • Enroll or change your medical, dental, or vision plans
  • Enroll or de-enroll dependents
  • Enroll or re-enroll in Health/Dependent Care FSA
    • Annually, it is required that you enroll in the Health/Dependent Care FSA.
Detailed information summarizing benefit plans and changes can be found in the 2018 Benefits Highlights Booklet.

How to Enroll
Faculty and staff can log in to Employee Self-Service November 1-22 to make benefits elections. To enroll in your 2018 Flexible Spending Account (FSA) for Health/Dependent Care, access the BenefitExpress portal at loyolaexpress.com.
  • For more information on how to complete open enrollment and how to access employee self-service, please review the information tab below.
  • If applicable, don't forget to complete the online Spousal/LDA premium and tobacco use premium certifications by November 22, 2017 through Employee Self-Service. The spousal/LDA premium and tobacco use premium acknowledgements will be presented to you in employee self-service while you go through annual open enrollment and at the time you elect/confirm your health insurance coverage.
Reminder
The annual open enrollment period is your opportunity to take dependents off of selected plans without documentation. If you are adding dependents to your coverage for the first time, please make sure that you provide the proper dependent verification documentation for each new dependent you add. In addition, Human Resources will need your dependent's full legal name and social security number. The type of verification documentation needed can vary by dependent type:
  • Please refer to page 7 of the 2017 Benefits Booklet for more details.
  • NEW in 2017: Documentation regarding dependents can be uploaded in Employee Self-Service during Open Enrollment. You no longer need to submit the information to benefits@luc.edu.
Revised: August 16, 2017
