Documents for Requesting Your PHPAC Letter

How Do I Request My Committee Letter Be Written?

So, you've submitted your applications to medical/dental schools and are ready to have your Committee Letter written and released to the appropriate application services. To do so, you'll need a variety of documents as PDFs on your PHAD profile to do so.

To have your letter written, you'll need all of the documents under either "First Time Applicants" or "Re-Applicants" uploaded to PHAD in addition to the documents listed under at least one of the application services listed below. Once all required documents have been uploaded to PHAD, don't forget to e-mail PHAdvising@luc.edu to notify the Committee that you're ready to have your letter written!

For First Time Applicants

Applicants applying to health professional schools for the first time are required to complete and upload the following documents as PDFs to their PHAD profile:

For Re-Applicants

Applicants requesting an addendum to a previous Committee Letter are required to complete and upload the following documents as PDFs to their PHAD profile:

For Applicants Utilizing AMCAS

Applicants wishing to have their Committee Letter/Addendum released to AMCAS must upload the following documents from AMCAS to their PHAD profile:

  • Completed AMCAS Application: This must show a "submission date" and will display all information used in application to allopathic schools
  • AMCAS Letter ID Form: This form will display a barcode near the upper margin

For Applicants Utilizing AACOMAS

Applicants wishing to have their Committe Letter/Addendum released to AACOMAS must upload a completed AACOMAS Application, which will display all information used in application to osteopathic schools.

For Applicants Utilizing AADSAS

Applicants wishing to have their Committe Letter/Addendum released to AADSAS must upload a completed AADSAS Application, which will display all information used in application to dental schools.

For Applicants Utilizing TMDSAS

Applicants wishing to have their Committe Letter/Addendum released to TMDSAS must upload a completed TMDSAS Application, which will display all information used in application to Texas medical/dental schools.

For Applicants Applying to Caribbean or International Schools

Applicants wishing to have their Committee Letter/Addendum released to any Caribbean or International health professional school should be in contact with the schools' to which you are applying to inquire to which e-mail address the Committee Letter/Addendum should be sent. Please simply add the schools' names and corresponding e-mail addresses in the appropriate fields on the Release of Letters Form.
