How to Upload your Vaccination Card

Thank you to the more than 14,000 Loyolans (more than 11,000 students!) who have uploaded their COVID-19 vaccination cards! Students who have not uploaded their vaccination cards or received approval for an exemption by August 6 will not be able to access campus buildings when the fall semester begins.

  1. Open the Loyola mobile app and select the “Loyola Health” function. From there, you will see a login page. Log in using your UVID and password.
  2. After logging in, select “Upload Vaccination” on the top menu.
  3. Select the vaccine type (Pfizer, Moderna, or Johnson & Johnson), enter the date of your first and/or second doses, and upload your vaccination card.
  4. After entering all information, click “Submit” to save the vaccination card.
