Additional forms are required each semester for Note Taking, Alternative Text, and Testing

If you are approved for Note Taking as an accommodation, you must complete the Note Taker Request Form prior to the start of each semester. This will notify SAC that you are an active student and you wish to utilize this accommodations; SAC will provide you with an access code for our note taking program: Sonocent. As a reminder, notes are meant to be supplemental, and they do not exempt you from taking your own notes or attending class. Please click here for the Audio Notetaker Activation.
If you are approved for Alternative Text (audio or PDF) as an accommodation, you must complete the Alternative Text Request Form prior to the start of each semester. This will notify SAC that you are an active student and you wish to utilize this accommodations; SAC will start the process of locating your books. Please note that this process may take a few weeks, so it is in your best interest to submit this form early. In the event that we are unable to obtain an alternate format of your text, you will need to discuss an alternative method with Disability Specialist. As a reminder, it is still expected that you purchase a copy of your text book. Publishers will require proof of purchase before providing an alternative text; please have your receipts available.
