Description:

This elective provides students the opportunity to be exposed to a wide variety of clinical and non-clinical situations encountered while providing care to children with complex health needs. The elective takes place two 12-bed transitional care facilities, bridging hospital discharge and home. Students will take part in the management of children with complex health needs in the context of an interprofessional care team that includes physicians, advanced practice nurses, nursing, social work, and others to provide a coordinated care plan. Students will learn and participate in the development of treatment plan options, monitoring, cardiorespiratory support and integration of physiology into daily clinical practice. http://almosthomekids.net

Method of Evaluation: Students will be evaluated on the basis of clinical performance and observation. Students will complete pre- and post-rotation assessments and have required readings. Students will also complete an independent study project. The course directors will determine the final grade.
