Be sure to submit your VA Enrollment Certification Request Form. The forms for , , and are available online. Submit completed forms via email to vetfinaid@luc.edu or in-person at the Student Services HUB in the Sullivan Center. You MUST submit this form before we can certify your enrollment with the VA for each term.
