The following forms and other resources are general forms that may or may not be used by a specific internship class. Check with the Academic Internships Program Manager if you are unsure which forms are best suited to properly document your students' work.
NOTE: Since Fall 2011, students and instructors in academic internship courses are required to document and approve their internship in LOCUS. Detailed instructions on this new procedure are below. We ask that this is done at the beginning of the semester.
