Become an Illinois Responder

Obtaining reciprocity to become an Illinois Responder

Currently certified in another state?  Not a problem.  Click here to download the application to obtain reciprocity in the state of Illinois.

Please be sure to follow ALL the steps listed. You will need to include copies of the following with your application:

1. Your current EMT licensure (state and/or national if applicable)
2. Your current CPR certification card
3. A letter of good standing from your current EMS system (if you have never worked in an EMS system, simply attach a letter stating why)
4. Child support declaration form (even if you do not have to pay child support)

Once all the above has been completed, you can send your application to the Department of EMS at Loyola's Lakeshore Campus:

We will then process your application internallly and forward in to the Illinois Department of Public Health for approval.
