All Club Sports will be using LUCommunity for forms, communications and overall organization of their club. 

Club Sports Agreement & Release (Waiver)

Submitted before participating in any club sport tryout, practice, game, etc. 

 

Club Sports Event Request (travel & events)

Submitted for any event your club is participating in (home game, away game, travel, fundraiser, etc.) 

 

Club Sports Hotel Purchase Request 

Submit this form when your club is traveling and requires overnight accommodations in the area. 

 

Club Sports Purchase & Payment Request

Submitted when your club needs to purchase or pay for anything (equipment, official’s fees, tournament fee, invoices, new uniforms, etc.) 

 

Club Sports Reimbursement Form  

Submitted when you need to be reimbursed for gas, tolls, etc. 

 

Club Sports Certified Driver Form  

Submitted in order to be approved as a driver of rental vehicles for club travel 

 

Club Sports Personal Vehicle Form 

Submitted every trip by anyone driving their own, personal vehicle for club travel 

 

Club Sports Tabling/Room Space Request Form

Submitted for clubs requesting a space on campus 2 weeks prior to their event (Example: Classroom needed for club meeting). 

 

Club Sports Injury Report (due 48 hours post injury)

Submitted by Club Sports Safety Officers.
