PLEASE NOTE:
The information below applies only to Stritch School of Medicine physicians-in-training or alumni. If you are a Loyola student or alumnus, please contact the University's Registration and Records office.
We cannot accept email requests for transcripts unless they include an attached PDF of a completed/signed transcript request form. We respond to faxes and regular mail in the same time frame.
VERIFICATION OF MEDICAL DEGREE/CERTIFICATION LETTERS
- There is NO fee for this service.
- Certification letter requests can be emailed, faxed or mailed with a SIGNED and DATED release form.
VERIFICATION OF RESIDENCY PROGRAM COMPLETION
- Residency Program verifications are completed by the Graduate Medical Education Office. Please call (708/216-4533) or email.
DENTAL VERIFICATION LETTERS
- For dental school verification letters for DDS graduates, call (312) 915-7221 or email.
