  • An explanation of the reason for exemption in your own words;
  • A description of the religious principle that guides your objection;
  • An indication of if this religious belief constitutes an objection to all immunizations or only this vaccine; if it is not an objection to all immunizations you must explain the religious basis for not receiving this particular immunization;
  • A statement that the student is aware they will be prohibited from attending their elective in the case of a vaccine-preventable outbreak or exposure.

*If not fully vaccinated (two weeks after your last dose) by the start of your rotation at Loyola, you will be unable to attend your scheduled elective course.

Rotations will be cancelled for physicians-in-training who have not been vaccinated or who do not submit the required documentation in VSLO.

FLU VACCINE REQUIREMENT DETAILS:

All students, physicians and staff at Loyola University Medical Center are required to be vaccinated for the seasonal flu. It is mandatory for all visiting students participating in electives at Loyola (and affiliated sites) to show proof of seasonal flu vaccination, beginning with the November elective rotations.

You will not be allowed to participate in your scheduled elective unless you are vaccinated and will be required to show proof of your seasonal flu vaccine before you can begin your elective. Please provide the documentation of your vaccination to your elective coordinator upon arrival at your elective service.

Acceptable documentation includes a signed statement from the vaccine provider giving the date, type of vaccine, name and address of the provider, the manufacturer, lot number, and dose.
