Wellness Program

New Wellness Program for 2016

The 2016 Wellness Program will feature financial and health wellness opportunities on campus. Planned events include educational programs, walks/runs, and Blue Cross Blue Shield rewards to assist you in leading a healthy lifestyle and financial wellness. Benefit-eligible employees who participate and complete five wellness activities, and submit the rewards form, are eligible for a $100 wellness reward. Employees are required to complete various wellness activities from January to May 2016. Wellness activities or services rendered in 2015 WILL NOT count towards the 2016 wellness incentive. The dates and times of various activities will be added to our website on a monthly basis. This way employees will complete activities over the course of 4-5 months and not all at once. The goal is to get you involved in your health on a regular basis.

How the 2016 program works: Complete at least 5 Wellness Activities between January and May of 2016 in order to be eligible for the reward. Wellness requirements will vary. Employees enrolled in our Loyola Advantage BCBS PPO program will have a form that they need to complete order to receive the wellness incentive for 2016. Employees who are benefit eligible but have opted out of the Loyola Advantage Program but are currently enrolled in a another Insurance program are also eligible to participate and are required to complete TWO forms.

General Preventive Care Services: Preventive care services are covered at 100% when you are enrolled in the Loyola Advantage PPO Plan and are covered 100% by the Loyola Advantage PPO Plan, when using a network physician. Please keep in mind that if any diagnoses is made during the wellness exam, the wellness exam is no longer covered at 100% For information about preventive care and wellness exams covered at 100% and if you need a general understanding of Preventive care see the links below.

Loyola Advantage BCBS PPO Participants

If you are currently enrolled in the Loyola Advantage BCBS PPO program you will be required to complete 5 wellness activities 3 of which can be done via the BCBS Well On Target wellness portal http://www.wellontarget.com/ Only employees who are enrolled in the Loyola Advantage Blue Cross Blue Shield (BCBS) PPO Health Plan will have access to participate in the Well on Target® activities. You will also have access to monthly challenges on the 1st of every month. You must enroll in the monthly challenge with in the 1st week in order to have continued access to the current months challenge It is always best that you retain proof that you have completed each wellness activity, health risk assessment, preventive care service, monthly challenge, and online course however proof that you have completed each category will be submitted to Loyola directly. You must complete all activities along with the form below by 5.30.2016 in order to receive the wellness incentive.

NON Loyola Advantage BCBS PPO Participants

If you are a benefit eligible employee but participate in a Health Insurance plan outside of Loyola University you can participate in the well initiative as well. You are also required to complete 5 wellness activities. You will need to complete and submit a 2016 Wellness Program Reward Form for NON Loyola Advantage BCBS Participants and a Wellness Activity Certification Form.

2016 Wellness Activities: Receive Preventive Care Services, attend Wellness Seminars, participate in Well on Target® Monthly Challenge, Well on Target® Health Risk Assessment and Well on Target® Online Courses.

Common Preventative Care Services Include:

Annual Physicals, Immunizations, Breast Exams, Mammograms, Colonoscopy, Prostate Exams, Mammograms, STI Testing, Smoking Cessation, Obesity Screening and Counseling, Pap Smears, Domestic Violence screening and Counseling, Screenings for Diabetes and other lab procedures. For more information on wellness exams and exams covered at 100% please take a look at our benefit booklet or contact BCBS directly.

Wellness Seminars: 2016 Wellness Seminars are provided by Perspectives and Transamerica. Each seminar will count as a wellness activity. Please note that dates and times below are subject to change and attendance will be taken at each seminar.
