Appeal forms and all supporting documentation must be submitted at least four weeks prior to the end of a term.

  • 2020-2021 Forms (includes Fall 2020, Winter 2020, J-Term 2021, Spring 2021, and Summer 2021)
  • 2021-2022 Forms (includes Fall 2021, Winter 2021, J-Term 2022, Spring 2022, and Summer 2022)

Students should login to our secure server using their Loyola username and password in order to submit Financial Aid documents. If parents or others would like to submit documents on a student’s behalf, the student may grant them access through the secure upload link.

Documents can also be dropped off in-person at the HUB desk in the Sullivan Center, faxed to 773-508-3397, or mailed to:
