Appeal forms and all supporting documentation must be submitted at least four weeks prior to the end of a term.

  • 2018–2019 Forms (includes Fall 2018, Winter 2018, J-Term 2019, Spring 2019, and Summer 2019)
  • 2019-2020 Forms (includes Fall 2019, Winter 2019, J-Term 2020, Spring 2020, and Summer 2020)

Students should login to our secure server using their Loyola username and password in order to submit Financial Aid documents. If parents or others would like to submit documents on a student’s behalf, the student may grant them access through the secure upload link.

Documents can also be dropped off in-person at the HUB desk in the Sullivan Center, faxed to 773-508-3397, or mailed to:
