Report Anonymously

This form is to be used for anonymously reporting a suspected disclosure of sensitive personal information.

Please provide as much detail as possible regarding the circumstances in which the sensitive data was exposed, disclosed, or otherwise improperly used. Any information provided will only be viewed by the Loyola Information Security & Compliance team.

This form does not request your name or email. To keep your report anonymous, avoid identifying yourself in the incident details.

Describe what happened, when it happened, and the systems or information involved. Do not include passwords or sensitive data.
