Program Requests/Forms
Are you looking to put on a health related program? The Wellness Advocates are a great addition to your programming needs! We can present to your group (most popular option), help design a program, or help facilitate your discussions. If you would like to request our assistance, please fill out and return the Wellness Advocates Program Request Form. The Wellness Advocates will accept requests for virtual and in-person presentations. Zoom can serve as a backup option as well if your group cannot secure a room for the presentation, or if turns out to be safer to hold the event online.
After you have downloaded and completed the form, email the form to wellnessadvocates@luc.edu and you will here back regarding your request shortly.
We do ask that requests be sent at least 14 days prior to your intended program date (if possible) in order to plan accordingly.
