Care Management and Referrals

The Wellness Center offers Care Management to any Loyola student who has a behavioral health or medical concern.  The Wellness Center provides short-term mental health counseling for a variety of issues and conditions. If you need longer term care, specialized care, care outside of business hours, or if you would prefer to seek treatment off campus, you will be offered a referral to care in the community. Care Management can assist you in navigating through the healthcare system to find the appropriate care. You may obtain care management services by being referred by a Wellness Center staff member or by the Office of the Dean of Students. You can schedule a care management appointment through the Wellness Center's online scheduling system. You can also call the Wellness Center at 773-508-2530, select option 3, and leave a voicemail. One of our staff will call you back as soon as possible, and help you schedule an appointment with care management. 

What happens during a care management session 

You and the Care Manager will work on identifying any concerns you may be having.  This is a brief appointment to get to know you better and gather the needed information to ensure that you are being referred to an appropriate provider.

Next Step

You and the Care Manager will review your insurance product. Since there are many different insurance products and plans, you and the Care Manager may call the insurance company together.  This may be a good time to review your current insurance policy details. Here is some advice in how to use your insurance to obtain mental health services.

Referral Process  

Once all of your concerns have been discussed, the Care Manager will assist you in finding appropriate referrals to providers in the community. You will then be provided a list of community providers that accept your insurance. 

Making phone calls to community providers can be challenging. The Care Manager will always offer the option to assist you in making these phone calls.  Once an appointment has been scheduled with a provider, the Care Manager will ensure that all of your needs are met and that you are properly connected to check in by providing follow up phone calls.
