COVID-19 Emergency Pay Request Form
Loyola University Chicago staff employees will be temporarily eligible to access additional COVID-19 Days once all Paid Time Off (PTO) accruals are exhausted. This includes Personal/Family Friendly Days, Sick and Vacation accruals with your Manger/Supervisor approval. As previously communicated, to ensure the greatest degree of flexibility, staff may now use any of their paid time off accruals, including personal/family friendly days, sick, and vacation time during the coronavirus period for pay continuity. Reminder: Exempt employees must always use PTO accruals in full day increments only. 

SPECIAL PROCEDURES DURING COVID-19 PERIOD: Eligible employees should complete the COVID-19 Emergency Pay Request Form and return the completed form to benefits@luc.edu
