COVID-19 Emergency Pay Request Form 
Loyola University Chicago announced that staff employees will be temporarily eligible to access up to 15 days of additional Paid Time Off during the Coronavirus (COVID-19) period once all Paid Time Off accruals are exhausted. This includes Personal/Family Friendly Days, Sick and Vacation accruals with your Manager/Supervisor approval.  As previously communicated, to ensure the greatest degree of flexibility, staff may now use any of their paid time off accruals, including personal/family friendly days, sick, and vacation time during the coronavirus period for pay continuity. 
 
SPECIAL PROCEDURES DURING COVID-19 PERIOD: Eligible employees should complete the COVID-19 Emergency Pay Request Form and return the completed form to benefits@luc.edu
 
New 2020 Federal Form W-4
Due to federal tax law changes that took place in 2018, the IRS recently released a revised Federal Form W-4 for 2020 in December 2019. Employees hired in 2020 or current employees who previously completed a 2019 or earlier form and want to make a change to their withholding status in 2020 must use the revised form. This form can be found in the "Government" section below.
 
Human Resources Website Security Initiative
Human Resources and the Financial Systems departments worked with ITS in June 2019 and February 2020 to secure high risk documents on our website to avert fraudulent activity and to reduce University risk. Some forms and instructions that specifically deal with sensitive information are now secured on our website so only anyone with a Loyola login (UVID) and password are able to access those documents. This means that some of the HR and Finance forms can only be accessed through secure URLs. Redirect links ensure that the original forms are not accessible without login credentials.
